Semaglutide vs. Tirzepatide: What Is the Difference?
Semaglutide and tirzepatide are different prescription medications: semaglutide acts on the GLP-1 receptor, while tirzepatide acts on both the GIP and GLP-1 receptors. Both are available in FDA-approved products for chronic weight management in eligible patients, and neither is right for every patient.
Mechanism
Semaglutide mimics GLP-1, a hormone involved in appetite, fullness, and blood glucose regulation. Tirzepatide combines GLP-1 activity with activity at the GIP receptor, a second hormone pathway involved in metabolism.
FDA-approved products
Each brand has its own approved indications, and compounded versions are not FDA approved.
- Semaglutide: Wegovy® (chronic weight management) and Ozempic® (type 2 diabetes)
- Tirzepatide: Zepbound® (chronic weight management) and Mounjaro® (type 2 diabetes)
Side-effect profiles
Both medications commonly cause gastrointestinal effects such as nausea, diarrhea, vomiting, or constipation. Both labels include a boxed warning about thyroid C-cell tumors and list other serious risks that should be reviewed with a physician.
Clinical considerations and eligibility
Eligibility depends on factors such as body mass index, weight-related conditions, personal or family history of certain thyroid cancers, pancreatitis history, pregnancy plans, current medications, and prior treatment response. Clinical studies report differences in average weight loss between medications, but individual results vary.
